Include: Patient name, DOB, what the concern is, and your return phone number.  

The second fastest is using HIPPA compliant app called OhMD. Once you set that up, no more faxing is needed. If you would like to receive a text, download the app, then indicate you would like to use this on the first faxed message and you will receive an “invite” from me.

To refer a patient:

Fax the last 3 office notes, face sheet, medication list, and any imaging reports of the head and/or cervical spine. After they are reviewed and determined to be within my scope of practice, the patient will be notified. The patient will complete a new patient questionnaire that must be returned before they are scheduled. When that is received, we call them to schedule their new patient visit.

If you are a pharmacist:

You will need to fax your questions. The fax comes directly to my an “inbox.” It does not sit on a fax machine. No one is available to answer the phone and no one returns your voicemail messages. If a patient is waiting at the pharmacy, you will need to tell them to come back. I encourage you to teach them to call before they come or to download your app that notifies them when their prescriptions are ready.
